Terms of Service & Waiver

In consideration of being permitted to participate in classes, programs, events, workshops, personal training sessions, and the use of equipment and facilities provided by Revive Studio ("Studio"), I acknowledge and agree to the following:

Assumption of Risk

I understand that participation in fitness activities, including but not limited to Reformer Pilates, Mat Pilates, Strength Training, Boxing, Yoga, Yoga Sculpt, and other fitness-related classes, involves inherent risks. These risks may include, but are not limited to, muscle strains, sprains, falls, equipment-related injuries, illness, property damage, and other injuries that may occur during or after participation.

I voluntarily assume all risks associated with participation in Studio activities and accept full responsibility for any injury, loss, or damage that may result.

Release and Waiver of Liability

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Revive Studio, its owners, instructors, employees, independent contractors, volunteers, representatives, agents, sponsors, and affiliates from any and all claims, demands, actions, causes of action, damages, losses, liabilities, costs, or expenses arising out of or related to my participation in Studio activities, including those resulting from ordinary negligence.

I understand that Revive Studio is not responsible for injuries, illnesses, property loss, or damages that may occur during or after participation in any class, program, or event.

Medical Responsibility

I certify that I am physically able to participate in fitness activities and have consulted a physician if necessary regarding my ability to engage in exercise. I understand that I am solely responsible for monitoring my own physical condition and for stopping activity if I experience pain, discomfort, dizziness, or other symptoms.

I agree that I am solely responsible for any medical expenses, treatment, or care resulting from injuries sustained during or after participation.

Equipment and Safety

I acknowledge that I have received or will receive instruction regarding the safe use of Studio equipment, including reformers and other fitness equipment. I agree to follow all safety guidelines, instructions, and directions provided by Studio staff and instructors.

I understand that failure to follow safety instructions may increase my risk of injury.

Participant Representations

By signing below, I represent and warrant that:

  • I have read and fully understand this Waiver and Release of Liability.

  • I voluntarily agree to all terms and conditions contained herein.

  • I am not under the influence of alcohol, drugs, or any intoxicating substance that may impair my ability to participate safely.

  • I am not knowingly suffering from any condition that would make participation unsafe.

  • I am at least 18 years of age. If I am under 18 years of age, a parent or legal guardian must sign on my behalf.

Photo and Media Release

I grant Revive Studio permission to photograph or record me during classes and events and to use such images or recordings for marketing, promotional, and social media purposes without compensation. I understand that I may notify the Studio in writing if I do not wish to be photographed.

SMS Terms & Conditions

1. Program Description

By opting in, you agree to receive recurring automated text messages from revive studio fitness ("we," "us," "our") at the mobile number you provided. Messages may include [appointment reminders, order updates, account alerts, promotional offers, customer support follow-ups, etc. — customize to your actual use case].
